Job summary

Pathways Healthcare is seeking a registered nurse to join our in-home care team serving Boston and surrounding areas from our Westwood, MA office. You will administer medications, perform assessments, and develop plans of care with physicians and interdisciplinary teams to support patients at home with dignity.

The role requires an active RN license in MA, BLS, and WCC certification, plus geriatrics experience. We offer flexible schedules, competitive per visit rates, and a supportive culture.

Qualifications

• Active RN license in MA.
• WCC certification.
• Prefer 2+ years in geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication.
• Knowledge of healthcare laws and regulations.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of the care plan development.

Responsibilities:

• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State(s) where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
