Job summary

Pathways Healthcare is seeking a dedicated registered nurse to lead in-home care and rehabilitation for patients in the Stoneham area. You will perform assessments, develop care plans, and coordinate with physicians and interdisciplinary teams to deliver patient-centered home health services.

The role emphasizes geriatrics, preventive and rehabilitative nursing, and close collaboration with caregivers. A valid MA nursing license, WCC, BLS, and a reliable vehicle are required.

Qualifications

• Active nursing license in MA required.
• Active Wound Care Certification (WCC) required.
• Prefer 2+ years of nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and teams to create plans of care.
• Address inquiries from patients, caregivers and medical teams.
• Provide counseling and support to patients and caregivers.
• Supervise home health aides as required.
• Identify discharge planning needs and implement these as part of care plans.

Responsibilities:

• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
