Job summary

Lineville Health & Rehabilitation is seeking a compassionate LPN to join our wound care/treatment team on a Full-Time, Monday–Friday day shift. You will provide specialized wound care, coordinate with RNs and physicians, and contribute to improving resident outcomes in a long-term care setting.

This focused role offers opportunities to develop wound care expertise, document progress, and support infection prevention within a supportive interdisciplinary team.

Qualifications

• Current Alabama Licensed Practical Nurse (LPN) license in good standing.
• Strong clinical observation and documentation skills.
• Excellent communication and organizational abilities.
• Compassionate, resident-centered approach.
• Ability to work effectively with the nursing team.
• Long-term care, rehabilitation, or wound care experience preferred.
• Previous treatment nursing experience is a plus.
• Wound care certification preferred but not required.

Responsibilities

• Provide wound and skin treatments according to physician orders and established protocols.
• Monitor wounds and skin conditions and report changes appropriately.
• Document treatments and resident responses accurately.
• Participate in routine skin and wound rounds.
• Help identify residents at risk for skin breakdown.
• Support preventive measures, including repositioning and pressure relief.
• Collaborate with RNs, physicians, and interdisciplinary team members.
• Assist with care planning related to skin integrity.
• Maintain treatment supplies and communicate needs.
• Reinforce wound prevention and skin care practices with nursing staff and CNAs.
• Support infection prevention and quality improvement efforts.
• Promote resident comfort, dignity, and healing.
