Job summary

Lineville Health & Rehabilitation is seeking an RN Wound Care / Treatment Nurse to lead wound prevention, treatment, and skin‑integrity efforts on a Monday–Friday day shift. You will coordinate care with physicians, nursing leadership, and frontline staff to ensure prevention, timely treatment, and accurate documentation.

The position emphasizes hands-on assessment, writing care plans, monitoring progress, and educating CNAs about skin integrity.

Qualifications

• Current Alabama RN license in good standing.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Compassionate, resident-centered approach.
• Ability to collaborate with physicians and interdisciplinary team.
• Long-term care, rehabilitation, or geriatric nursing experience preferred.
• Previous wound care or treatment nursing experience preferred.

Responsibilities

• Perform comprehensive skin and wound assessments.
• Provide wound treatments according to physician orders and established protocols.
• Monitor wounds and document healing progress.
• Conduct routine skin and wound rounds.
• Identify residents at risk for skin breakdown and implement preventive interventions.
• Monitor turning, repositioning, pressure relief, and other prevention measures.
• Collaborate with physicians and interdisciplinary team members regarding treatment plans.
• Participate in care planning related to wounds and skin integrity.
• Maintain accurate and timely clinical documentation.
• Monitor wound care supplies and treatment needs.
• Educate nurses and CNAs regarding skin integrity, prevention, and wound care practices.
• Serve as a clinical resource for wound and treatment questions.
• Participate in quality improvement, infection prevention, and regulatory readiness related to wound care.

Position Overview

The RN Wound Care / Treatment Nurse coordinates and provides wound and skin care while collaborating with nursing leadership, physicians, frontline staff, and the interdisciplinary team.
