Job summary

Five Oaks Rehabilitation and Care Center in Concord, North Carolina is seeking a compassionate Wound Care Registered Nurse (RN) for day shifts (7a-3p). You will conduct wound assessments, administer care, and coordinate with the interdisciplinary team to optimize healing outcomes.

Qualifications

• Current, unencumbered LPN license in North Carolina.
• Wound care experience or certification preferred.
• Strong knowledge of wound healing, infection control and pressure ulcer prevention.
• Excellent communication and collaboration skills.
• CPR/BLS certification required.

Responsibilities

• Conduct comprehensive wound assessments and develop individualized treatment plans.
• Administer wound care treatments including dressing changes, debridement, and documentation.
• Monitor wound healing progression and communicate findings with the interdisciplinary team.
• Collaborate with providers, nursing staff, and therapists to deliver coordinated care.
• Educate staff, patients, and families on wound care management and prevention.
• Maintain accurate, detailed, and timely clinical records in the EMR.
• Adhere to infection control and safety protocols in wound care delivery.
