Job summary

Integrated Wound Care in Minnesota is seeking a compassionate Nurse Practitioner to join our team on a part-time basis. You will assess wounds, develop personalized treatment plans, and collaborate with a multidisciplinary team to improve resident outcomes in skilled nursing, long-term care, and rehabilitation facilities.

You will document care efficiently in electronic medical records, educate patients and families on wound care, prevention, and management, and monitor progress to adjust plans

Qualifications

• Active license in the state of employment.
• Minimum of 1 year of experience in wound care.
• Ability to work independently and as part of a team.
• Exceptional critical thinking and decision-making skills.
• Excellent verbal and written communication skills.
• Strong time management and organizational abilities.
• Comfortable with technology and electronic medical records.

Responsibilities

• Conduct wound assessments and provide personalized treatment plans for patients.
• Work collaboratively with a team of healthcare professionals to ensure optimal patient care.
• Complete documentation accurately and in a timely manner.
• Educate patients and their families on wound care, prevention, and management.
• Monitor patient progress and adjust treatment plans accordingly.
