Job summary

Integrated Wound Care in Minnesota is seeking a compassionate Nurse Practitioner to join our part-time wound care team. You will evaluate patients in skilled nursing, long-term care, and rehabilitation facilities and develop individualized treatment plans to improve outcomes.

Responsibilities include wound assessments, collaborating with a care team, timely documentation, patient and family education, and progress-based plan adjustments.

Qualifications

• Licensed NP in state of employment with current credentials.
• Minimum 1 year of wound care experience.
• Ability to work independently and in a team.
• Strong critical thinking and decision-making abilities.
• Excellent verbal and written communication.
• Strong time management and organization.
• Proficient with EMR systems and healthcare technology.

Responsibilities

• Conduct wound assessments and provide personalized treatment plans for patients.
