Vantage Health Partners in Columbus, Ohio, is seeking an experienced Wound Care Nurse Practitioner for a part-time, on-site role two days per week across three central-Ohio facilities. You will perform assessments, debridement, and develop individualized treatment plans to promote healing and enhance patient quality of life.

Qualifications

• Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited NP program.
• Active Ohio APRN license.
• National NP board certification (FNP, AGNP, or AGACNP).
• Active DEA registration.
• Demonstrated wound assessment and staging expertise.

Responsibilities

• Perform comprehensive wound assessments, including measurement, staging, and documentation.
• Conduct sharp and non-sharp debridement procedures as clinically indicated.
• Develop, implement, and adjust individualized wound care treatment plans.
• Evaluate patient progress and modify interventions to promote healing.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.
• Educate patients and caregivers on wound prevention, dressing care, and self-management.
• Prescribe medications, dressings, and wound care supplies per state regulations.
• Maintain accurate, timely clinical documentation in the EMR.
• Identify and manage complications such as infection and delayed healing.
• Ensure compliance with clinical protocols, infection control standards, and regulatory requirements.
• Participate in quality improvement initiatives related to wound care outcomes.
