Job summary

CareMed is seeking a Wound NP to deliver specialized wound care services to residents in long-term care facilities.

The role requires an OH NP license, board certification, and experience with wound care in geriatric patients. Work is performed within an interdisciplinary team, coordinating care with primary physicians and nursing staff.

Qualifications

• Current, unrestricted NP license in OH.
• Board Certification in appropriate specialty.
• Clinical experience in wound care and geriatric settings.
• Comfort diagnosing/staging pressure injuries per NPIAP.
• Experience with wound care products and DME.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances as allowed by OH law.
• Order and review treatments and diagnostic tests; follow up as needed.
• Coordinate with nursing home staff on condition changes and care plans.
• Collaborate with the primary care physician to optimize care.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
