Job summary

CareMed is seeking a part-time Wound Care Nurse Practitioner to provide wound services for residents in long-term care facilities in Ohio. The role requires a current OH NP license, board certification, and wound care experience, with DEA licensure optional.

You will work within an interdisciplinary team, using EMR systems to coordinate care, monitor wounds, and collaborate with primary care physicians to optimize treatment plans.

Qualifications

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• Coordinate with care plans as needed.
• Collaborate with the primary care physician.
