Job summary

Healogics is seeking an experienced WCC LPN to provide wound-related nursing care under the supervision of a Registered Nurse or Provider. You will follow hospital policies and CMS guidelines while supporting vein clinics and discharge planning.

The role requires an active LPN/LVN license, 2+ years in SNF/LTACH, and preferred wound care experience. Strong communication, organization, and teamwork are essential in a fast-paced outpatient setting.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice.
• Two (2) or more years’ experience as LPN/LVN, including SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under RN/Provider direction per BLS and hospital protocols.
• Assist in vein clinic as trained; support intake and discharge.
• May assist with case management under RN supervision, per state regulations.
• Gathers patient information and supplies for RN/Provider.
• Report quality of care issues to Clinical Nurse Manager; communicate with team.
