Job summary

Healogics is seeking an LPN/LVN to join its wound care team and provide care under the supervision of an RN or Provider. The role includes assisting with vein clinics, intake/discharge tasks, and coordinating with the care team to ensure high-quality patient outcomes.

Qualifications include a current LPN/LVN license, 2+ years in LPN/LVN roles (SNF/LTACH preferred), and basic BLS, with wound care experience favored. The position emphasizes teamwork and patient safety.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN or LVN license in the state of practice and maintained throughout employment.
• Two or more years’ experience as LPN/LVN, including SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under the direction of RN/Provider according to policies.
• Assist in vein clinic after training.
• May assist with intake and discharge.
• May assist case management under RN direction following state regulations.
• Gather patient information and necessary equipment.
• Report quality of care issues to Clinical Nurse Manager.
• Communicate with team members regarding interventions to ensure quality care.
• Perform hyperbaric therapy-related duties after training.
