Job summary

Healogics, Inc. is seeking a Wound Care Center Licensed Practical Nurse (LPN) in Westlake, OH. You will provide wound-related patient care under an RN or provider, support vein clinic activities, assist with intake/discharge, and help with case management as needed.

The role requires a practical nursing license, 2+ years' LPN/LVN experience (incl. SNF/LTACH preferred), and BLS competency. A patient-focused, team-oriented approach is essential.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN or LVN license in the state of practice.
• Two or more years’ experience as LPN/LVN, including at least 1 year in SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Under RN/Provider direction, provides wound-related patient care per BLS and hospital protocols.
• Assist in vein clinic as trained.
• May assist with intake and discharge.
• May support case management under RN supervision.
• Gather patient information and necessary equipment.
• Report care quality issues to the Clinical Nurse Manager.
• Communicate with team about therapeutic interventions to ensure quality of care.
• Perform hyperbaric oxygen therapy-related duties after training.
