Job summary

HealWell of Texas, LLC seeks an APRN/PA to join our wound care team across private residences, skilled facilities, and outpatient clinics. You will assess wounds, develop treatment plans, perform debridement within scope, and use advanced wound therapies.

Travel is required within the service area, with autonomy balanced by team collaboration. The ideal candidate has TX APRN/PA licensure, national certification, strong assessment skills, and experience in wound care.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current, unrestricted APRN/PA license with prescriptive authority in Texas.
• Current national board certification.
• Valid driver's license and ability to travel within the assigned service area.
• Strong clinical assessment and decision-making skills.
• Ability to work independently and manage a mobile patient schedule.
• Excellent communication, organization, and documentation skills.
• Comfortable providing care to medically complex patients in home, facility, and outpatient settings.
• Two or more years of experience as an advanced practice provider preferred.
• Previous wound care experience preferred but not required for the right candidate.
• Wound care certification preferred; may be required to obtain certification within six months of hire.

Responsibilities

• Perform comprehensive patient and wound assessments.
• Develop and manage individualized wound care treatment plans.
• Accurately assess, measure, stage, and document wounds and wound characteristics.
• Perform wound care procedures within scope of practice, including debridement as appropriate.
• Utilize advanced wound care products, therapies, and treatment modalities.
• Assess vascular status and perform or interpret vascular screening, including ABI/TBI when indicated.
• Coordinate referrals, testing, and follow-up care to overcome barriers to healing.
• Collaborate with physicians, nursing staff, home health, facilities, patients, and families.
• Educate patients and caregivers on wound care, prevention, nutrition, and offloading.
• Maintain accurate, timely clinical documentation and compliance with HIPAA.
• Travel to assigned patient locations as required.
