Job summary

HealWell, LLC is seeking a motivated Wound Care APRN/PA to join our growing wound care team. The role involves delivering specialized wound care across diverse settings, including private residences, skilled nursing facilities, long-term care facilities, and outpatient clinics.

The provider works independently while collaborating with patients, families, and facility staff to develop individualized treatment plans focused on wound healing, limb preservation, and quality of life.

Qualifications

• Requires NP or PA program graduate and active licensure in Oklahoma.
• National board certification is required.
• Two+ years of AP provider experience preferred.
• Wound care certification preferred; if not certified, eligible to obtain within six months.

Responsibilities

• Perform comprehensive patient and wound assessments.
• Develop and manage individualized wound care plans.
• Assess, measure, stage, and document wounds.
• Perform wound care procedures within scope, including debridement as appropriate.
• Utilize advanced wound care products and therapies.
• Coordinate referrals, testing, and follow-up care.
• Educate patients and caregivers on wound care and prevention.
• Maintain accurate clinical documentation.
• Travel to patient locations as required.
