Frontier Behavioral Health is seeking a WISe Youth Peer Support Specialist to provide targeted peer support within home and community settings. The role includes completing CANS assessments, coordinating with enrollment teams, and engaging families to promote recovery and stability for youth in Medicaid-eligible programs.

The position requires flexibility for a 24-7 schedule, travel to community sites, and collaboration with the WISe team to ensure child-centered care and family involvement.

Competenze

• Must have been a youth who experienced emotional and behavioral challenges and have been in recovery for one or more years.
• Must hold an active Certified Peer Support Specialist credential or be eligible for trainee credential within 90 days of start date.
• High school diploma or equivalent is required.
• Must have an unrestricted valid driver's license and willingness to use personal vehicle for work.

Mansioni

• Provide education about WISe program, service intensity, and eligibility criteria to referrals.
• Perform initial CANS assessments to determine WISe eligibility.
• Complete WISe service enrollment and ensure timely enrollment documentation.
• Provide education and referrals for services to clients not eligible for WISe.
• Maintain client records with confidentiality and accuracy.
• Collaborate with multi-disciplinary team to develop treatment objectives and interventions.
• Offer ongoing peer support to families and promote recovery and empowerment.
