Job summary

Humana Inc. is hiring a Case Manager for the Inclusa team to support Wisconsin Family Care members in navigating care and living independently.

You will build relationships, coordinate services, and help members reach personal health and independence goals from La Crosse/Sparta, WI, with travel up to 40% and a 40-hour workweek. TB screening and a driver-safety program are part of the role.

Qualifications

• Bachelor's degree in human services or related field with at least 1 year serving frail elders or adults with disabilities, or a Bachelor's in another field with 3+ years such experience.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services per MCP in the least restrictive environment.
• Conduct face-to-face social assessments with members upon enrollment and at least every six months, usually at the member's home.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical support services including social integration, community resources, employment, housing.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide member education.
• Maintain accurate documentation including case notes and MCP updates.
