Job summary

Four Seasons is seeking a qualified RN Case Manager to coordinate nursing care for a caseload in Macon County and surrounding areas. You will supervise staff and deliver direct care within licensed practice, ensuring adherence to regulations and quality standards.

The role requires coordination with the Clinical Team Leader, a 40-hour week with on-site responsibilities and occasional weekend/holiday coverage. Full-time and part-time options may be available.

Qualifications

• Two years of clinical experience preferred.
• Valid NC RN license.
• Valid Driver’s License and proof of automobile insurance.

Responsibilities

• Coordinates the nursing plan of care and provides direct nursing care.
• Manages a caseload of approximately 15 patients with varying acuity.
• Teach, supervise, and evaluate staff delivering nursing care.
• Collaborate with other health care providers to determine patient-centered plans.
• Provide teaching and counseling about the patient’s health.
