Job summary

Details This is seeking a coordinator to support and guide employees through the WellU program at the Osher Center for Integrative Health on campus. You will collaborate with employees to develop individualized wellness plans based on Health Risk Assessment results and serve as a liaison to connect employees with programs and services for timely access and positive outcomes.

Qualifications

• Bachelor’s degree in Health Care Administration, Business, Communication, Nursing or related field.
• Experience with health programs and data management to support program evaluation.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Experience in oncology, multidisciplinary clinics, and/or working with high-acuity, complex patients may be preferred.

Responsibilities

• Health Risk Assessment Management: Monitor HRA dashboards and reports to identify participant needs and support timely follow up.
• Program Enrollment and Orientation: Assist employees in navigating the WellU program and its benefits; Facilitate enrollment for new participants; May conduct orientation sessions.
• Tailored Care Pathway Coordination: Review HRA results, provide one-on-one consultations, develop personalized wellness plans, and follow up to ensure engagement.
• Resource Coordination: Coordinate with departments and wellness providers to ensure a seamless experience for employees.
• Program Promotion and Outreach: Promote WellU through channels, organize wellness events, and create informational materials.
• Data Collection and Reporting: Maintain program records and prepare regular reports on utilization and outcomes.
• Collaboration and Teamwork: Work with Director and staff to align initiatives with wellness goals.
