Spectrum Retirement Communities is seeking a part-time Wellness Nurse to support resident health through clinical monitoring and collaboration with care teams in our Ohio community.

Responsibilities include admission screening, care plan adherence, medication administration, and coordinating with external providers while maintaining regulatory compliance. A current LPN/RN license and geriatric experience are required.

Qualificações

• Working knowledge of state regulations, policies, and procedures, dictated for compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three (3) years'experience in assessment and geriatric nursing preferred
• A big smile, eagerness to learn, and compassion for our residents

Responsabilidades

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures
