Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse in Illinois for a part-time role. You will support residents with clinical monitoring, assist in coordination with other care staff, and ensure adherence to care plans and physician orders.

The position requires an LPN or RN license, state registration, and experience in geriatric assessment. A compassionate, team-focused approach is essential for high-quality resident care.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years' experience in assessment and geriatric nursing.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures
