Job summary

Spectrum Retirement Communities, LLC. is seeking a part-time Wellness Nurse to support residents’ health through clinical monitoring and coordination with care teams. You will assist in admissions, review care plans, and administer medications in accordance with physician orders.

The role emphasizes collaboration with nurses, caregivers, and external providers in a patient-centered environment. A valid LPN/RN license and geriatric nursing experience are required.

Qualifications

• Graduate from an accredited school of practical nursing.
• Active LPN/RN license with state registration.
• 3 years of geriatric nursing and assessment experience.

Responsibilities

• Assist in coordination of resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians' orders
• Coordination of care with outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain knowledge of state regulations, policies, and procedures
