Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse to support residents' health through ongoing clinical monitoring and collaboration with care teams. The role requires coordinating admission care, administering medications per physician orders, and helping ensure regulatory compliance in a fast-paced, compassionate environment.

Qualifying candidates will have a practical nursing credential, a current RN/LPN license, and at least three years' geriatric assessment experience.

Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three (3) years' experience in assessment and geriatric nursing preferred.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians’ orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations, policies, and procedures
