Job summary

NIVA Health is seeking a Wellness Case Manager to guide patients through their wellness journey from initial conversation to ongoing treatment plan engagement. This role blends health, wellness, and sales to support patient engagement and outcomes in a clinic setting.

You will partner with providers and leadership to keep patients informed, engaged, and connected to services that meet their health goals, driving measurable results in a collaborative environment.

Qualifications

• Track record guiding people toward a purchase, membership, or programme decision in wellness, health coaching, or related field.
• Naturally personable, energetic, and confident in one-on-one conversations.
• Excellent communication and rapid rapport building.
• Motivated by measurable goals linked to patient engagement and enrollment.

Responsibilities

• Build trust-based relationships with patients from first interaction.
• Guide patients through wellness and therapy offerings aligned to goals.
• Support patient engagement, retention, and treatment plan participation.
• Coordinate follow-ups, scheduling, and patient communications.
• Create a seamless, elevated patient experience at every touchpoint.
• Track KPIs tied to engagement and enrollment in new services.
• Support new wellness initiatives and service launches within the clinic.
• Collaborate with providers and clinic staff.
