Job summary

Senior Living Residences is seeking a Wellness Coordinator for Concord Park. You will manage the Resident Care schedule, assign caregivers, and provide direct personal care when needed. You’ll also lead engaging activities and document care using the EHR system.

Applicants should hold CNA or CHHA certification, be comfortable with scheduling tools, and communicate effectively with residents and staff to ensure safe, compassionate care in a home-like setting.

Qualifications

• CNA or CHHA certification required.
• Ability to learn scheduling and EHR systems.
• Effective communication with residents and staff.

Responsibilities

• Manage Resident Care Department schedule and assignments.
• Provide RCA coverage when needed.
• Spend quality time providing direct personal care.
• Lead or join resident activities.
• Track care tasks using electronic health records.
• Assist housekeeping with light duties.
• Escort residents to meals and dining services.
