Qualifications

• High School Diploma or GED (or equivalent).
• Valid Michigan driver's license with a good driving record.
• Reliable and ongoing transportation.
• 1 year of related experience preferred.
• Must complete all required background clearances.
• Background check including fingerprinting and MILTC screening.

Responsibilities

• Coordinate and communicate with the support team to maintain health and wellness.
• Coordinate medical and dental appointments and aftercare.
• Communicate healthcare information to all members of the Support Team.
• Review healthcare progress data and report on outcomes toward goal areas.
• Support recovery-based practices and person-centered planning as a shared decision-making process.
