Responsibilities include reviewing healthcare data, reporting outcomes, and facilitating person-centered planning for individuals served.

Qualifications

• High School Diploma / G.E.D or equivalent.
• Valid and unrestricted State of Michigan Driver's License with a good driving record.
• Must have reliable and ongoing transportation.
• 1 year of related experience preferred.
• Must successfully complete all required background clearances.

Responsibilities

• Coordinate and communicate with the support team to maintain, promote and monitor individuals' health and wellness.
• Coordinate all medical and dental appointments and aftercare.
• Communicate healthcare-related information to all members of the Support Team.
• Review healthcare progress data and report on outcomes toward goal areas.
• Support culturally competent recovery-based practices and person-centered planning as a shared decision-making process.
