Job summary

Luminos Hospice is seeking a skilled On-Call LPN to join our compassionate team. This weeknight role provides nursing care to patients in their homes during evenings, coordinating with the Interdisciplinary team, patients, families, and referring agencies.

Responsibilities include delivering high-quality care, making sound clinical decisions, and ensuring smooth coordination of care in a fast-paced hospice setting. Current LPN license, CPR, and driver’s license required.

Qualifications

• Current State LPN license.
• Current driver’s license.
• Current CPR Certification.

Responsibilities

• Provide nursing care for patients in their residences.
• Coordinate care with the Interdisciplinary team, patients and families.
• Coordinate care with referring agencies.
• On-call duties during weeknights/evenings.
