Job summary

Luminos Hospice is seeking a skilled On-Call Nurse to provide high-quality evening care to patients in their homes. You will administer nursing care, coordinate with the interdisciplinary team, families, and referring agencies, and respond to patient needs during weeknights.

Current State RN or LPN license, CPR, and a valid driver’s license are required. Two years of clinical nursing experience, with hospice experience preferred. Strong communication and independent decision-making are essential.

Qualifications

• Current State RN or LPN license.
• Current driver’s license.
• Current CPR Certification.
• Two years of nursing in a clinical setting; hospice experience preferred.

Responsibilities

• Provide nursing care to patients in their homes during weeknights.
• Coordinate care with the Interdisciplinary team, patients, families and referring agencies.
• Participate in care coordination responsibilities and ensure quality patient outcomes.
