Luminos Hospice is seeking a skilled On-Call Nurse to deliver high-quality nursing care for patients in their homes during off-hours and weekends.

Responsibilities include coordinating care with the Interdisciplinary Group/Team and families, ensuring smooth handoffs, and maintaining clinical standards in a home-health setting. A current RN or LPN license is required, with valid driver’s license and CPR certification.

• Current state RN or LPN license.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Excellent verbal and written communication skills.
• Strong attention to detail and organizational habits.

Mansioni

• Provide nursing care for patients in residence during off-hours and weekends.
• Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
• Assume responsibility for and participate in care coordination.
