Job summary

The Care Team Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our night shift team. You will deliver skilled nursing care under the direction of the Registered Nurse, monitor patient conditions, and support patients and families through the hospice journey.

This on-site role offers 10-12 hour shifts, a 3-4 nights per week schedule, and comprehensive benefits. Prior hospice or home health experience is preferred, with EMR documentation and interdisciplinary collaboration

Qualifications

• Active LPN license.
• Hospice/home health experience preferred.
• Strong clinical assessment, communication, and patient care skills.
• Ability to work independently while collaborating with a team.
• EMR experience preferred.

Responsibilities

• Provide skilled nursing care under RN direction per patient care plan.
• Monitor patient conditions and communicate updates to care team.
• Assist with symptom management, medications, and comfort measures.
• Document patient care and observations in EMR.
• Collaborate with physicians, RNs, caregivers and the team to deliver hospice care.
