Job summary

Luminos Hospice is seeking a skilled On-Call Nurse to provide high-quality evening care to patients in their homes. You will administer nursing care, coordinate with the interdisciplinary team, families, and referring agencies, and respond to patient needs during weeknights.

Current State RN or LPN license, CPR, and a valid driver’s license are required. Two years of clinical nursing experience, with hospice experience preferred. Strong communication and independent decision-making are essential.

Job Overview

Luminos Hospice is seeking a skilled and compassionate On-Call Nurse to join our team. As an On-Call Nurse, you will be responsible for providing quality care to our patients in the evenings. Primary functions are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.

Requirements

Licensure

Current State RN or LPN license.

Current driver’s license.

Current CPR Certification.

Experience

Two (2) years' experience as a Nurse in a clinical care setting and one (1) year full-time hospice preferred.

Skills
