Job summary

Luminos Hospice is seeking a skilled On-Call LPN to join our team. This weekend on-call role requires providing nursing care for patients in their homes during weekends, coordinating with the Interdisciplinary Group/Team, patients, families, and referring agencies. The position emphasizes independent assessment, timely decisions, and compassionate end-of-life care.

Requirements include an active LPN license, CPR certification, and a valid driver's license. Prior hospice experience is preferred.

Job Overview

Luminos Hospice is seeking a skilled and compassionate On-Call LPN to join our team. As an On-Call nurse, you will be responsible for providing quality care to our patients during the weekends. Primary functions are to administer nursing care for patients of all ages in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.

Requirements

Licensure

Current State LPN license.

Current driver’s license.

Current CPR Certification.

Experience

Two (2) years' experience as a nurse in a clinical care setting and one (1) year full-time hospice preferred.
