Job summary

Willow Creek Health Care Center is seeking an RN Weekend Supervisor to coordinate resident care during weekend hours, serving as a clinical liaison among residents, families, physicians, and payers. The role ensures high-quality, compliant care aligned with each resident’s Plan of Care.

The ideal candidate holds an active RN license with 2+ years of clinical nursing experience and a preferred Bachelor’s degree in Nursing or Social Work.

Qualifications

• Active RN license required; LVN/LPN considered.
• Bachelor’s degree in nursing or social work preferred.
• Minimum two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs.
• PCC experience preferred.
• Ability to read and interpret procedures and policies.
• Effective communication with residents, families, and staff.

Responsibilities

• Coordinate and oversee resident care during weekend operations.
• Communicate plans of care and treatment protocols regularly.
• Participate in morning stand-up meetings for interdisciplinary coordination.
• Monitor resident status and adjust care interventions as needed.
• Ensure accurate clinical documentation and timely charting.
• Promote collaboration among interdisciplinary team members and families.
• Oversee admissions, discharges, and referrals as appropriate.
• Maintain compliance with regulatory standards and facility policies.
