Qualifications

• Active RN license required; Bachelor’s degree in Nursing preferred.
• Minimum two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs.
• Certified Case Manager (CCM) certification a plus.

Responsibilities

• Coordinate and oversee resident care during weekend operations.
• Communicate Plans of Care with residents, families, and the care team.
• Attend morning stand-up meetings to facilitate interdisciplinary communication.
• Ensure timely, accurate clinical documentation.
• Oversee admissions and discharges and coordinate referrals.
