Gentiva is seeking an experienced Registered Nurse (RN) Clinical Liaison in Raleigh, NC to coordinate hospice admissions, educate families, and maintain strong relationships with referral sources. The role emphasizes compassionate patient care, clinical evaluations, and efficient intake processes.

Ideal candidates hold an RN license with 3–5 years hospice experience (BSN preferred) and CPR certification. Travel to hospitals and facilities is required to support discharge planning and community

Qualificações

• Licensed RN with 3–5 years hospice experience.
• BSN with 5+ years hospice experience preferred.
• Current RN license to practice in the employment state.
• CPR certification is required.

Responsabilidades

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and others.
• Assess patient eligibility for hospice services via records review and clinical evaluation.
• Educate patients and families on hospice care components and processes.
• Complete necessary documentation and coordinate intake/admission per legal and payer requirements.
• Serve as liaison between referral sources and the hospice team for seamless handoffs.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians.
• Build relationships with referral partners and identify new outreach opportunities.
