Job summary

PruittHealth @ Home in Georgia is seeking an experienced Registered Nurse for weekend Baylor Home Health assignments. The role provides skilled nursing care under physician supervision, with focus on start of care, care coordination, and patient education.

You will supervise Home Health Aides and LPNs, collaborate with the interdisciplinary team, and complete timely documentation (OASIS, SOC, ABNs) at the point of care.

Qualifications

• RN licensure in state of practice is required.
• Minimum 1 year of RN experience.
• Valid driver's license and reliable transportation.
• Liability insurance to complete home visits.
• Current CPR certification.
• Nursing degree from an accredited college or university.

Responsibilities

• Provides skilled nursing care & interventions per physician supervision.
• Supervises HHAs and LPNs on the care team.
• Collaborates with interdisciplinary team to develop and manage the patient care plan.
• Performs start-of-care assessments and clinical outcomes monitoring.
• Completes documentation at point of care (OASIS, SOC/Admissions, Recertifications, ABNs).
• Ensures patient rights and adheres to care plans and regulations.
