Job summary

PruittHealth @ Home in Georgia-Cumming is seeking a Registered Nurse (RN) for Weekend Baylor – Home Health to provide skilled nursing care under physician supervision and coordinate care at patients' homes. New pay per point model offers top pay for higher-effort visits.

Minimum of 1 year RN experience, active RN licensure, CPR certification, and a valid driver’s license are required. BSN preferred with home health experience and strong teamwork in a patient‑centered setting.

Qualifications

• Minimum Education Required: Nursing degree from an accredited college or university, or graduate of an accredited/approved RN program.
• Minimum Experience Required: Minimum of one (1) year of RN experience; Valid driver’s license; Reliable transportation; Liability insurance to complete home visits.
• Minimum Licensure/Certification Required By Law: Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice; Current CPR certification.
• ADDITIONAL QUALIFICATIONS: BSN preferred; Acute care experience with adult or aging populations; Home health and/or skilled nursing experience.

Responsibilities

• Provides competent nursing care & interventions to home health patients according to state and industry standards.
• Supervises care provided by Home Health Aides and LPNs on the care team.
• Collaborates with interdisciplinary team to develop, integrate, and manage the patient care plan of care.
• Performs field visits to evaluate start of care and conduct clinical outcomes monitoring.
• Completes documentation timely, accurately, and at the point of care (OASIS, SOC/Admissions, Recertifications, ABN’s).
