Job summary

PruittHealth is seeking a Home Health RN Case Manager for weekend Baylor in Georgia. This role offers autonomy, meaningful one-on-one patient care, and work-life balance through flexible scheduling and strong clinical support.

As a Home Health RN, you will perform skilled nursing visits at patients’ homes, develop care plans, and collaborate with the multidisciplinary team while guiding LPNs and aides. AI-assisted documentation may be used on an iPad, with OASIS experience a plus.

Qualifications

• Active, unrestricted RN license
• Valid driver’s license and reliable transportation
• CPR certification
• 1+ year of RN experience (home health or acute care preferred)

Responsibilities

• Provide skilled nursing visits in the patient’s home
• Complete assessments, start-of-care visits, and care plans
• Collaborate with the interdisciplinary care team
• Supervise LPNs and Home Health Aides
