Qualifications

• RN license active in the state of practice.
• Minimum of one year of RN experience.
• Liability insurance to complete home visits.

Responsibilities

• Provide skilled nursing care and interventions to home health patients per plan of care.
• Supervise care provided by HHAs and LPNs assigned to the care team.
• Collaborate with interdisciplinary team to develop and manage the patient care plan.
• Demonstrate competency in home health care specific patient skills.
• Conduct field visits to evaluate start of care and monitor outcomes/quality.
• Utilize company teaching materials for patient/family education.
• Document patient health progress using required documents and timelines.
• Maintain patient rights and adhere to physician-approved care plans.
