Qualifications

• Nursing degree from an accredited college or university or graduate of RN program.
• Minimum of one year RN experience; liability insurance to complete home visits.
• Current, active RN licensure in state of practice.
• BSN preferred; acute care/home health experience preferred.

Responsibilities

• Provides competent nursing care & interventions to home health patients per plan.
• Supervises care provided by HHAs and LPNs.
• Collaborates with interdisciplinary team to develop and manage the patient care plan.
• Conducts field visits to start of care and monitor outcomes.
• Documents using required technology and timelines (OASIS, SOC, ABN, etc.).
• Ensures patient rights and adheres to care plan and compliance program.
• Maintains licenses and certifications and meets regulations.
