Job summary

BayCare Health System is seeking a part-time Hospital Care Coordinator-RN Case Manager to support Wesley Chapel Hospital. The role includes linkage, monitoring, planning, and advocacy to maximize patient functioning, with a focus on effective discharge planning and resource coordination.

The position requires an RN license, 2 years in acute care, and 2 years as a registered nurse. ACM/CCM preferred. Weekend schedule, 16 hours weekly, with Saturday and Sunday shifts.

Qualifications

RN license required.
Associates degree in nursing required.
Bachelor degree in nursing preferred.
ACM (Case Management) and CCM (Case Manager) preferred.
Minimum 2 years in Acute Care and 2 years as a Registered Nurse.

Responsibilities

Provides linkage, monitoring, planning and advocacy for our population to achieve maximum functioning.
Coordinates discharge planning through assessment, referral and interdisciplinary planning.
Manages assigned caseload, adjusting to patients' needs and priorities.
Promotes resource utilization for quality, cost-effective outcomes.
