Job summary

BayCare Health System is seeking a part-time Hospital Care Coordinator-RN Case Manager to support Wesley Chapel Hospital. The role includes linkage, monitoring, planning, and advocacy to maximize patient functioning, with a focus on effective discharge planning and resource coordination.

The position requires an RN license, 2 years in acute care, and 2 years as a registered nurse. ACM/CCM preferred. Weekend schedule, 16 hours weekly, with Saturday and Sunday shifts.

Qualifications

• RN license required.
• Associates degree in nursing required.
• Bachelor degree in nursing preferred.
• ACM (Case Management) and CCM (Case Manager) preferred.
• Minimum 2 years in Acute Care and 2 years as a Registered Nurse.

Responsibilities

• Provides linkage, monitoring, planning and advocacy for our population to achieve maximum functioning.
• Coordinates discharge planning through assessment, referral and interdisciplinary planning.
• Manages assigned caseload, adjusting to patients' needs and priorities.
• Promotes resource utilization for quality, cost-effective outcomes.

The Hospital Care Coordinator-RN Case Management Responsibilities Include

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
• Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Ability to adjust to changing priorities in a fast-paced environment.

Position Details

• Location: Wesley Chapel Hospital
• Status: Part time, 16 hours per week.
• Schedule: Saturday & Sunday - 8:00 AM - 4:30 PM
• Weekend Requirement: Every
