Job summary

BayCare Health System is seeking a Care Coordination Specialist for Winter Haven Hospital. This part-time role (16 hours/week) supports Saturday and Sunday, 8:30 AM – 5:00 PM, with no on-call requirements.

You’ll work within an interdisciplinary team to optimize clinical, quality, and financial outcomes by coordinating care and discharge planning. The ideal candidate brings a background in social work or healthcare, with preferred medical social work or LPN experience, and strong collaboration

Qualifications

• Bachelor’s degree in social work, psychology or healthcare field is required.
• HS/GED with LPN license acceptable in lieu of degree.
• Experience in medical social work or LPN desirable.

Responsibilities

• Function as key member of interdisciplinary care team to optimize outcomes.
• Provide comprehensive care coordination and discharge planning.
• Identify barriers including social determinants affecting transitions of care.
• Coordinate services, referrals, and post-acute resources timely.
• Support compliant length of stay and value-based reimbursement initiatives.
