Job summary

BayCare in the Tampa Bay area is seeking a part-time Hospital Care Coordinator-RN Case Management to support patient care across discharge planning and care coordination.

The role requires an RN with at least 2 years in acute care and 2 years as an RN, with ACM/CCM preferred, and involves Saturdays and Sundays 8:00 AM to 4:30 PM. PTO, 401k, bonuses and discounts are provided.

This position emphasizes advocacy, resource linkage and timely care transitions within a large system.

Qualifications

• Requires RN licensure with at least 2 years in Acute Care.
• Associates degree in Nursing required; Bachelor’s preferred.
• Experience in discharge planning and case management desirable.

The Hospital Care Coordinator-RN Case Management responsibilities include:

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
• Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Ability to adjust to changing priorities in a fast-paced environment.
• Status: Part time, 16 hours per week.
• Schedule: Saturday & Sunday - 8:00 AM - 4:30 PM
• On Call: No

Certifications and Licensures:

• Required RN (Registered Nurse)

• Preferred CCM (Case Manager)

Education:

• Required Associates of Nursing or
