Job summary

BayCare in the Tampa Bay area is seeking a part-time Hospital Care Coordinator-RN Case Management to support patient care across discharge planning and care coordination.

The role requires an RN with at least 2 years in acute care and 2 years as an RN, with ACM/CCM preferred, and involves Saturdays and Sundays 8:00 AM to 4:30 PM. PTO, 401k, bonuses and discounts are provided.

This position emphasizes advocacy, resource linkage and timely care transitions within a large system.

Qualifications

• Requires RN licensure with at least 2 years in Acute Care.
• Associates degree in Nursing required; Bachelor’s preferred.
• Experience in discharge planning and case management desirable.

Responsibilities

• Provides linkage, monitoring, planning and advocacy to maximize functioning.
• Understands resources to promote quality, cost-effective outcomes.
• Manages caseload with attention to individual needs and preferences.
• Coordinates discharge planning through assessment and interdisciplinary planning.
• Adapts to changing priorities in a fast-paced environment.
