AccentCare, Inc. is seeking an RN Weekend Baylor to serve as a Home Health Case Manager in Humble, Texas. The role blends direct and indirect patient care and requires coordinating with CHHAs and CNAs to follow the patient care plan.

The position offers a Friday–Sunday schedule, full-time hours, and a salary of $83,000–$102,000 depending on experience, with benefits and travel requirements as described in the posting.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of operation.
• One (1) year experience as an RN.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with an automobile that is insured and able to travel to all business locations.
• Must have Geriatric Home Health and OASIS experience.

Responsibilities

• Assess nursing needs of assigned patients and implement the individualized care plan.
• Promote and maintain optimal client care; diagnose and treat health problems.
• Make the initial nursing evaluation visit and re-evaluate needs as required.
• Initiate and revise the plan of care as needed.
• Provide services within the nurse's scope of Practice Act.
• Assign, supervise and teach other clinical personnel.
• Initiate rehabilitative nursing procedures and inform physicians of changes in condition.
• Counsel and instruct clients and families in nursing needs.
