Job summary

AccentCare is seeking a Weekend Baylor RN Case Manager to join our Home Health team in the Austin area. This full-time role supports South Austin and surrounding areas, delivering high-quality patient care Friday through Sunday with a base salary reflected in a six-figure range including a $10,000 bonus.

You will coordinate patient care plans, lead CHHAs and CNAs, perform assessments, and adapt plans as patient needs evolve.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• One (1) year experience as an RN.
• Must possess and maintain valid CPR certification while employed in a clinical role.

Responsibilities

• Assessment of the status of nursing needs of assigned patients, implements the individualized patient care plan, evaluates the effectiveness of the care plan, and modifies as appropriate.
• Promoting and maintaining optimal client care. Diagnosing and providing treatment for human responses to actual or potential health problems.
• Makes the initial nursing evaluation visit and re-evaluates the client’s nursing needs as required.
• Initiates the plan of care and necessary revisions.
• Provides those services requiring substantial special skills within nurses scope of Practice Act.
• Assigns, supervises and teaches other clinical personnel, including written para-professional orders and supervision.
• Initiates appropriate preventative and rehabilitative nursing procedures.
