AccentCare Inc. is seeking a dedicated RN Weekend - Baylor to join our Home Health team in Humble, TX. This full-time role covers NE Houston and surrounding areas, with a Friday–Sunday schedule and a $83,000-$102,000 annual salary.

You will oversee patient care plans, coordinate with care teams, and ensure quality outcomes for pediatric to geriatric patients. Requirements include an active RN license, 1 year of RN experience, CPR certification, and OASIS experience.

Qualifications

• Active RN license in the state of operation.
• One year of RN experience.
• CPR certification required.
• Geriatric home health and OASIS experience preferred.

Responsibilities

• Assessment of nursing needs and implementing the individualized care plan.
• Promote and maintain optimal client care; diagnose health problems.
• Make the initial nursing evaluation visit and re-evaluate as required.
• Initiate and revise the plan of care as needed.
• Provide services within the nursing scope of Practice Act.
• Assign, supervise and teach other clinical personnel.
• Initiate preventive and rehabilitative nursing procedures.
• Inform physician and staff of changes in patient condition.
• Counsel and instruct client and family on nursing needs.
