AccentCare, Inc. is seeking a RN Case Manager for Weekend Baylor in the Austin South Office. The role is full-time, on-site, with a Friday–Sunday schedule and a salary range of $83,000 to $102,000 plus a $10,000 bonus.

Qualifications

• RN license in the state of operation.
• Minimum 1 year RN experience.
• CPR certification current.
• Must have geriatric home health and OASIS experience.

Responsibilities

• Assess patients and implement the individualized care plan.
• Provide direct and indirect patient care across age groups.
• Supervise and mentor CHHAs and CNAs in care tasks.
• Document patient care and adjust plans as needed.
